October 2025 expanded CARE HMIS across pediatric OP, inpatient workflows, pharmacy, lab, inventory, accounting, and the Kerala Palliative Care Grid's volunteer and dashboard capabilities.

October 2025 was the month CARE HMIS began moving from pilot presence into deeper departmental adoption at Sree Sudheendra Medical Mission Hospital.

The September launch had proved CARE could run in a live hospital environment. October focused on the next, harder layer: expanding coverage, digitizing more real workflows, standardizing data, and adjusting the platform based on live use by doctors, nurses, pharmacists, lab staff, store teams, accountants, and administrators.

Pediatric OP becomes structured digital care

The pediatric outpatient unit was activated on CARE. Doctors and nursing station teams were trained on outpatient workflows, patient documentation, and service request handling.

Existing pediatric questionnaires and assessment formats were digitized into CARE so clinical data could be captured in structured form. This is one of the most important shifts in an HMIS rollout: the system stops being only a patient registration and billing layer and begins representing specialty-specific clinical practice.

Pediatric workflows also tested whether CARE's questionnaire system could adapt to real departmental documentation without requiring custom software for every specialty. The answer shaped later work on templates, favorites, and extensible clinical forms.

Hospital-wide users, roles, and nursing stations

October brought broader user onboarding across the hospital.

Floor in-charges and nursing staff were trained on account creation, department allocation, nurse-doctor linkage, and shift adjustments. Nursing stations across floors were brought into CARE workflows. User credentials were created, departments assigned, and role-based access configured for nursing stations, OP desks, and support units.

This role configuration work is easy to miss, but it is central to healthcare software. A nurse, doctor, pharmacist, lab technician, receptionist, store manager, and accountant should not see or modify the same parts of the system. CARE had to support hospital-specific access while protecting governance and data integrity.

Inventory and product knowledge are rebuilt

The engineering work in October rebuilt the inventory module to improve stock visibility, data structures, and operational workflows for medicines, consumables, and equipment.

Product knowledge rendering was corrected so dosage forms, categories, pricing, and other attributes displayed consistently. Charge item definitions were standardized so they applied correctly when creating service requests or billing items. Inventory navigation was improved to reduce confusion.

At the hospital, the team worked with store management to review existing inventory workflows, identify stock-flow gaps, and improve stores operations. Updated stock lists for medicines, consumables, and implants were compiled, validated, and imported into CARE.

This work made the pharmacy and stores layer more reliable. In a hospital, stock data is not a background table. It affects dispensing, billing, procurement, expiry management, and financial reconciliation.

Lab standardization with SNOMED CT

The lab module continued to mature through workflow optimization and terminology mapping.

Activity definitions and observation definitions were updated with SNOMED CT coding for clinical standardization. The team manually reviewed the complete list of tests and mapped corresponding SNOMED codes and observation interpretations so diagnostic results could be classified more consistently.

This is the sort of work that makes an HMIS more than digitized paperwork. A lab report with structured observations and standard terminology is easier to query, interpret, compare, and integrate. It can support dashboards, interoperability, and future clinical decision-support workflows.

Inpatient workflows begin

October also marked the beginning of inpatient rollout.

Nurses across designated inpatient floors were onboarded. Training covered admission flow, vitals capture, order entry, medication administration, and handover documentation.

Inpatient care is a major leap in complexity compared with outpatient workflows. It involves shifts, beds, medication rounds, consumables, nursing notes, physician orders, discharge planning, and continuous documentation. Starting IP workflows meant CARE was moving into the operational core of the hospital, not merely its front desk.

Accounting and doctor fee configuration

The CARE-ERP accounting plugin was demonstrated to accounting and administrative teams. Multiple feedback loops helped align the plugin with hospital financial practices.

Doctor consultation fees were configured as charge items so billing could reflect the hospital's actual operating model. Patient account workflows improved through better transaction cancellation, discount calculations, invoice navigation, and financial error handling.

The accounting work reinforced a recurring HMIS lesson: clinical adoption and financial correctness have to mature together. A hospital may tolerate some UI friction during transition, but it cannot tolerate unreliable billing.

Kerala CARE gets volunteer workflows

The Kerala Palliative Care Grid also evolved in October.

A new version of CARE was released across the palliative grid with simplified nursing workflows, faster access to past encounters, and improved documentation flows. NHM state teams were trained on the updated workflows, and district-level doubt-resolution sessions helped teams transition.

The Volunteer Module was implemented, allowing nurses to assign patients to volunteers and record volunteer home-visit details directly within CARE. A Volunteer Visit Form supported structured documentation of volunteer activity.

This extended CARE's representation of palliative care beyond formal clinical staff. Community participation is part of Kerala's palliative care model, and the software had to make that participation visible without losing accountability.

The quality discipline begins to shift

October also introduced Playwright end-to-end testing. Early tests covered location creation, user schedule creation, and patient registration, and the team began refining workflows for automated testing, caching, and reporting.

This was a quiet but important platform move. CARE was now touching billing, inventory, bed management, queues, lab, pharmacy, palliative care, roles, forms, and hospital workflows. Manual testing alone could not protect that surface area.

Automated testing became part of making CARE deployable with confidence.

Live usage became the design review

October's work was not defined by one headline feature. It was defined by refinement under live pressure.

Pediatric OP, inpatient nursing, inventory, lab standardization, accounting, volunteer workflows, role management, keyboard interactions, multilingual support, and Playwright testing all moved in parallel. Around 100 pull requests landed during the month, while the team continued on-site hospital onboarding and statewide palliative support.

That is what a real HMIS rollout looks like after go-live. The product becomes a negotiation between platform consistency and local workflow truth. October showed CARE absorbing that negotiation without losing its core architecture.
